Submitting a new insurance claim

  1. Access the Forto help center through the following link: https://forto.atlassian.net/servicedesk

2. In case you do not have an account yet click on the “Need an account? Sign up.” option and follow the instructions in the mail sent to you.

3. Select the “Insurance - Create a new insurance claim” option.

4. Enter the details of the insurance claim

5. Sending out Jira form

After finishing all required fields, click the “Send” button at the bottom of the page. You will be redirected to a page with an overview of the filled out form.

Checking the status of current insurance claims

By clicking on the “Request” button in the top right corner, you can check the current status of your insurance claims.

Here you can view all of your requests associated with this account. You can use the filters to change the displayed requests (e.g. changing from “Open requests” to “Closed requests” to view already finished requests. You can view click on the “Reference” of the requests to view the status of the specific request.

Please use the comment function to enter additional information (e.g. uploading additional documents).
